Patients with both skin laxity and fat bulges in the lower eyelid require a subciliary approach. This is where an incision is made just below the lower eyelashes. This allows for skin tightening and fat repositioning to smooth out hollows and reduce puffiness. In some cases, lateral canthopexy is performed to stabilize the lower eyelid, preventing sagging or distortion. If upper eyelid ptosis is present, the procedure may include reattaching the levator muscle to enhance upper eyelid function and appearance.
